3. Is a detailed care plan designed, reviewed and updated for each resident?

4. Does the facility provide on-site physical, occupational and speech therapies?

5. Does the administrator and Director of Resident Care Services conduct regularly scheduled meetings and communications with residents and their families?

6. Does the staff know each resident by name?

7. Is the staff orientation and training specifically designed for caring for assisted living residents?

8. Is the management responsive and helpful with your questions and needs?

9. Is the facility part of a larger organization that has additional facilities and services to meet the needs of residents as their needs change?

10. Is the facility licensed?
